The Benefits of Our Plan
The Radiance Family & Cosmetic Dentistry Membership Plan is not dental insurance – and that’s why it’s a great option! When you choose our Membership Plan, you will love that there is no more hassling with complicated deductible and reimbursement schedules and no more pre-authorizations, premiums or yearly maximums. Instead, it is simply a way to receive quality dental care at a price you can easily afford.
- 1. Our membership plan provides the professional oral care you need at an affordable price!
- 2. Benefits start immediately and our membership runs for 12 months from the day you join.
- 3. Our membership plans are customized to your dental needs and preventative care is at no additional cost.
Annual Membership Plans
Child Plan (up to age 12)
Plan Includes:
- 2 Professional Cleanings
- 2 Periodic exams and oral cancer screenings
- 2 Fluoride treatments
- Routine digital x-rays
Adult Plan (ages 12+)
Plan Includes:
- 2 Professional Cleanings
- 2 Periodic exams and oral cancer screenings
- 2 Fluoride treatments
- Routine digital x-rays
Periodontal Plan (ages 12+)
Plan Includes:
- 3 Periodontal maintenances
- 2 Periodic exams and oral cancer screenings
- 3 Fluoride treatments
- Routine digital x-rays
Additional Savings
All plans include 20% off restorative treatment (such as fillings, crowns) and if needed or wanted 20% off additional cleanings; 15% off periodontal, root canals and surgery; 10% off removable appliances and implants
*Plan is determined based on your age or periodontal status.
PROGRAM DETAILS
- The Radiance Family & Cosmetic Dentistry Membership Plan is not a dental insurance and is not a qualified health plan under the Affordable Care Act. If you obtain dental insurance during the year, there is no refund to this plan.
- Payment for the plan is due at the time of signing up.
- Payment for treatment is due at time of service.
- The membership cannot be used if you have dental insurance, other offers, or discounts.
- Our Membership plan does not cover procedures completed by other dentist or specialists outside of our practice.
- Renewal payment is due on the first day of the anniversary month (membership runs on a 12-month period).
- Copay must be paid in full at time of service to qualify for any courtesy savings.
- Any appointments not used during the 12-month period will not be entitled to a refund.
